A conceptual model is simply a diagram that illustrates the various relationships between the factors that are relevant to the public health issue.

This conceptual modelling tool is intended to be used by specialists to develop a visual representation of a public health issue being addressed. It will assist in mapping the relationships between determinants and health outcomes. The tool provides information on when to construct a conceptual model and the steps involved.
