Your child must have a diagnosed developmental disability or a pending diagnosis to be referred to Peel Case Management. Anyone can make a referral provided there is parental consent.
- Call the Region of Peel at 905-799-7700.
- Tell them you’re calling about Peel Case Management.
- Your call will be transferred to a Public Health Nurse who will complete an “intake” form with you over the phone. All the information you provide will be kept confidential.
- Our Intake Coordinator will contact you to determine how we can help you.

The Region of Peel has a call centre for all Peel Health programs, including public health and long-term care. All inquiries will be answered by Public Health Nurses between the hours of 8 a.m. and 5 p.m. Monday to Friday. The nurses will approach each caller with sensitivity and ensure that the caller is provided with appropriate resources. All information will be kept confidential.
