Planning & Networking for Transition

The Passport Initiative offers a workshop for teens (14+) with developmental disabilities and their families to prepare them for the teen’s transition from school to adult activities.

This initiative will encourage families to develop a support team that includes siblings, contract workers, teachers, and friends in order to help their child make the transition from school to work and/or post-secondary activities in the community.

The youth will be encouraged to get involved with the creation of a complete and achievable plan for the future. Families and young people will also benefit from the contacts they make here, gaining information and sharing their “vision” with others.

At the end of training, your family will have a complete, achievable plan that your support team can put into action.
