If you live in Mississauga, Brampton or Caledon and have been placed on medical surveillance by Citizenship and Immigration Canada (CIC), follow these steps:
- Call Peel Public Health at 905-799-7700 and ask to speak to a public health nurse in the TB program.
- After talking to a public health nurse, print the Letter for Your Doctor and Doctor Report Form, and to take to your doctor.
