Applying for Services

Required Documents

Depending on individual circumstances, you may be requested to provide the following documents for each family member applying for assistance:

  • Verification of date of birth, e.g. birth certificate, passport, immigration papers, or certificate of Indian Status card
  • Social Insurance Number (SIN) card
  • Ontario Health Insurance Plan (OHIP) card
  • Proof of rent or home ownership, e.g. lease, rent receipts, mortgage papers, property tax papers, condominium fee receipts, utility bills, land title and deed
  • Peel Living residents are to provide their most recent Rental Charge Sheet
  • Pay stubs or written proof of income
  • Bank statement or passbook (must be up-to-date)
  • Divorce/separation papers, support agreements
  • Proof or documentation of child care costs
  • Employment Insurance (EI) stubs
  • Proof of investments (RRSP, bonds, term deposits, shares, etc.)
  • Proof of self-employment and all financial information for your business: registration, business bank account statements, proof of company assets, profit and loss statements
  • Previous year's Notice of Assessment (income tax return)
  • Doctor's note if you are physically or mentally ill and cannot work
  • A resume or a list outlining work history and educational background
  • Other documents as requested

The application process will be delayed if all of the necessary papers are not provided at the Initial Participation Agreement.
