Peel Outreach Program

"The Peel Outreach Program provides comprehensive mobile outreach services to vulnerable members of our community or those at imminent risk of becoming homeless in the Region of Peel. Our Team strives to provide these services in an effective, efficient and sensitive manner through a client-centred, holistic approach that will increase an individual’s personal capacity."

Our Vision

In addition to offering mobile outreach services, the Outreach Program endeavours to improve access to current services by bridging the gap between these transitional services and individuals in the community.

Community Partnership

The Outreach Program is a collaborative effort between the Region and three community organizations: the Canadian Mental Health Association (Peel Branch), Peel Addiction Assessment and Referral Centre, and St. Leonard’s House.

How We Can Help

The Peel Outreach Program Team consists of trained and caring professionals who are experienced in helping the most vulnerable or at-risk of becoming homeless. The Team can help with: Ontario Works applications for assistance and case management, health care, food, clothing, blankets, mental health, addiction, “street counselling” – assessment and support. The Team can also support individuals with referrals and advocacy for: housing, emergency shelter, medical care, employment, mental health supports and substance abuse supports.

Those We Have Helped (Jan. 2006 – Dec. 2008)

Total number of individuals supported: 1,281 with 5,507 outreach services, referrals and supports provided.

Challenges

The Peel Outreach Program Team needs your support with: connecting to people in need or at-risk, sharing knowledge about the program with your community members and support with charitable donations.
