What you need to bring with you

Immunocompromised individuals must bring 1 of the following to a clinic or pharmacy to receive your vaccine:

  • Referral form completed by doctor or nurse practitioner.
  • Current prescription package, label, or pharmacy receipt of an immunosuppressant medication listed by the Ontario government. The prescription must clearly label patient name, name of medication, date of dispensing, and name of prescribing doctor.
  • If you have a prescription for an immunosuppressant medication not listed by the Ontario government, you can contact your doctor or health care provider to receive a referral form for a third or booster dose of the COVID-19 vaccine.

Peel Public Health is working with local pharmacy and primary care partners to provide A limited supply of the Novavax and Janssen vaccines. Refer to more information about these vaccines.

Health Canada continues to recommend mRNA vaccines such as Comirnaty (Pfizer-BioNTtech) or Spikevax (Moderna).

The Janssen COVID-19 vaccines should only be used when an mRNA vaccine is declined and after informed consent. Janssen vaccines may be considered as a booster dose for individuals who have had the same vaccine as a first dose. Discuss your vaccine choices with a health care provider before booking an appointment.

To get vaccinated with one of these vaccines, do not make an appointment through Ontario’s vaccine booking system. Contact the following locations directly to register your interest and schedule an appointment.

Appointments are only open to residents of Peel.
