We (I and my partner) have never shared needles or equipment for drug or steroid use, tattooing or piercing.

If you answered, "YES" to questions 1, 2, 3, 4 or 5 - you're at almost NO risk for acquiring HIV/AIDS, hepatitis B or other STIs.


My partner and I use a latex barrier every time we have vaginal and/or anal intercourse.


My partner and I use a latex barrier every time we have oral sex.


My partner and I have both tested negative for HIV (AIDS). Neither my partner nor I shares needles or equipment for drugs, and we don’t have any other sex partners.

If you answered, "YES" to 6, 7 or 8 you're at low risk for all STIs, including HIV/AIDS. If you said, "YES" to 8 you may be at low risk for all STIs, including HIV/AIDS.


I’ve had vaginal and/or anal intercourse without a condom.


I’ve had oral sex without using a condom or latex barrier.


I (or my partner) had/have a sexually transmitted infection such as gonorrhea or chlamydia.


I don't know my partner's sexual history.


I know my partner has had unprotected sex with other people.


I’ve had sex with a man who’s had sex with another man.


I don't know if my partner has shared drug needles or equipment


I share needles and/or equipment for drug and/or steroid use, tattooing and/or piercing.


I had a blood transfusion before 1985 - or - I had sex with someone who had a transfusion before that time.

If you answered, "YES" to 9, 10, 11, 12, 13, 14, or 15, you're at risk for infections like HIV/AIDS, hepatitis B and other STIs.

If you said, "YES" to 15, 16, or 17, you may be at risk for HIV/AIDS and hepatitis B and C.
