Immunization Records for Child-Care Centres
Peel Public Health collects, reviews and updates immunization records of children in licensed child-care centres. Children attending licensed child care should be immunized for diphtheria, pertussis, tetanus, polio, measles, mumps, rubella, and haemophilus influenzae type B (Hib) according to the Ontario immunization Schedule.
Immunize on time
- Immunizations must be given at the right times to work. For example, the first dose of MMR (measles, mumps and rubella) must be given on or after a child's first birthday and the second dose at least 28 days after the first dose.
- Between four to six years of age your child needs a booster dose for diphtheria, pertussis, tetanus and polio.

Keep your child's immunization record up-to-date
- Your doctor does not report your child's immunization information to Peel Public Health.
- Update your child's immunization record with the child-care centre during registration and each time your child is vaccinated. Your child-care provider will send it to Peel Public Health.
- See your family doctor or call Peel Public Health if your child has not been immunized or you do not have documents that show your child has been immunized.

- A child who has been immunized outside of Ontario or Canada might need additional immunizations to meet Ontario's provincial immunization laws.
- Required immunizations to attend licensed child-care centres in Ontario are free through your family doctor.
- Peel Public Health will translate a child's immunization record into English for free.
- A child who does not have health insurance (e.g., OHIP) and needs an immunization record to attend a licensed child-care centre, should contact Peel Public Health for information.
