How to Request your Ambulance Call Report (ACR)

Lawyer Requesting ACR on behalf of a Client

There are three pieces of information a Law Firm will need to provide in order to obtain an ACR for their client:

  1. A general letter requesting an ACR and providing the necessary information as listed below:
    • Patient’s name
    • Patient’s date of birth
    • Date of the incident
    • Time of the incident
    • Location of the incident
    • If patient was transported and to which hospital
    • Any additional details pertaining to the incident
  1. A signed authorization form from the patient providing consent to release confidential information to your office.

  2. A cheque in the amount of seventy-five dollars ($75.00) made payable to “The Region of Peel”.

Please allow one to two weeks for your request to be processed. If you do not hear back from the PRPS Risk & Audit Department within the designated time-frame please contact our office to ensure your request has been received.

Once the ACR has been located, a copy of the report will be express mailed to your office.

Please note, in the event an ACR cannot be located, a letter will be mailed to the Law Firm and the cheque in the amount of seventy-five dollars ($75.00) will be returned.

Patient requesting their own Ambulance Call Report (ACR)

Patients who would like to obtain a copy of the ACR taken at the time of the incident can do so by providing the two necessary documents below:

  1. A general letter requesting an ACR and providing the necessary information as listed below:
  • Patient’s name
  • Patient’s date of birth
  • Date of the incident
  • Time of the incident
  • Location of the incident
  • If patient was transported and to which hospital
  • Any additional details pertaining to the incident
  1. A cheque in the amount of seventy-five dollars ($75.00) made payable to “The Region of Peel”.

Once the ACR has been located, depending on the method of correspondence you indicated, we will notify you that it is ready for pick up either by telephone or mail. At the time of pick up, you will be required to show two pieces of government identification (one must be photo ID). The identification will be photocopied for our records.

Please note, in the event an ACR cannot be located, a letter will be mailed to you and the cheque in the amount of seventy-five dollars ($75.00) will be returned.
