How to Request Your Ambulance Call Report (ACR)

Lawyer Requesting ACR on Behalf of a Client

There are three pieces of information a law firm needs to provide in order to obtain an ACR for their client:

  1. A general letter requesting an ACR, which provides the following information:
    • Patient’s name
    • Patient’s date of birth
    • Date of the incident
    • Time of the incident
    • Location of the incident
    • If patient was transported and to which hospital
    • Any additional details pertaining to the incident

  2. A signed authorization form from the patient providing consent to release confidential information to your office.

  3. A cheque in the amount of $75 made payable to The Region of Peel.

Please allow one to two weeks for your request to be processed. If you do not hear back from the PRPS Risk & Audit Department within this time, contact us to ensure your request has been received.

Once the ACR has been located, a copy of the report will be express mailed to your office.
