Health Babies Healthy Children screening form
This screening form is used to refer pregnant people, and families with infants and young children to this free program. Peel Public Health nurses provide in-person home visits.

Tools for Ontario's enhanced 18-month well-baby visit
Resources from McMaster University Health Sciences to help health professionals implement the enhanced 18-month visit in your practice, including the Looksee Checklist (formerly NDDS), Rourke Baby Record, and WHO Growth Charts.
