Routine vaccines
To order vaccines through Peel Public Health, fill out the vaccine order request form.

High risk program vaccines
To order vaccines under high risk programs, fill out the appropriate requisition form and fax to the number indicated on the form:
- Hepatitis A vaccine requisition
- Hepatitis B vaccine requisition
- High Risk Hepatitis B vaccine requisition
- Meningococcal vaccine requisition
- Human Papillomavirus (HPV) vaccine requisition for MSM

Influenza vaccines
Influenza vaccines are not available at this time. The vaccines will be available for order at the beginning of October for the 2024-2025 Influenza season.
